ABOUT YOUR VISIT
Preparation for your appointment
We are excited to welcome you to our practice! Once you have made an appointment with our office please ensure you bring a form of identification. This can be a license, passport, permanent resident card, or other identifying document such as a Global Entry card or SENTRI card. In addition to your form of identification you will need to have your insurance information. If possible bring your insurance card with you or email us an e-insurance card ahead of time. Finally, if you are coming in for a specific concern and have previously visited a doctor please bring in your test results or email an electronic copy to our office ahead of time to ensure Dr. Yao has all the necessary information.
